Habilitation Aide - Mifflin County

UCP is looking for dedicated, compassionate, and caring individuals to provide 1:1 home and community habilitation, companion, and/or respite services to individuals with developmental, intellectual, and physical disabilities in Mifflin County. DSP’s provide community integration, skills training, support, and opportunities that promotes greater independence, personal growth, and community involvement. Paid training and mileage & expense reimbursement.

Requirements

  • High School Diploma or GED
  • At least 18 years of age
  • Experience with individuals with disabilities preferred
  • Effective communication skills
  • Basic math, reading, and writing skills
  • Valid PA driver’s license with clean driving record; reliable, insured personal vehicle and willingness to transport (required)
  • Successful completion of required background checks – FBI, criminal, child abuse, and driving

Duties

Essential Duties and Responsibilities:

  • Implement Individual Support Plan (ISP)
  • Provide support services for the individual to more fully participate in their home and community setting
  • Provide training in daily living, communication, community living, money management, household responsibilities, and other skills training as identified in the ISP
  • Provide companionship and socialization
  • Provide personal care assistance
  • Provide support to enable full participation in school, day training programs, and/or their community
  • Participate on interdisciplinary team; collaborate and cooperate with involved agencies, family members, and other service providers; participate in meetings and provide relevant information
  • Assist individual to plan activities in the home and community in accordance with the ISP
  • Provide transportation for community activities/outings, day program, school activities or scheduled appointments in staff’s personal vehicle
  • Participate in in-service training
  • Complete and submit all daily progress notes and paperwork in a timely and professional manner
  • Be professional, dedicated, reliable, and able to work independently
  • Perform other duties as assigned
