Four-tiered Transitional Program
Intro text here
Tier 1: Alternatives Adult Day Program
Alternatives Adult Day Programs are our more traditional center-based programs offered in 10 locations. These programs offer a wide range of on-site activities and an option for community activities for program participants.
Tier 2: Neighbors Adult Day Program
Neighbors Adult Day Programs offer a more contemporary approach with a greater emphasis on community-based activities. Daily outings provide program participants with either recreation or volunteer opportunities. The Neighbors programs are held in three locations.
Tier 3: Stepping Stones
The Stepping Stones Program is designed to empower each individual to focus on, and make progress in, three particular skill sets: communication and socialization, giving back through volunteerism, and vocational training.
Tier 4: Pathways
Pathways is a two-year, curriculum-based program that places a keen focus on employment outcomes.
Residential Transition Program
Pathways Academy is an intensive 9-18 month curriculum-based, residential training program that helps adults with intellectual disabilities learn the skills needed to live with minimal supports in the community.
This housing option fills the gap between in-home care and group home care. We currently have 10 people living in individual apartments, while we maintain a staff apartment in the complex. This arrangment affords the residents a greater degree of freedom and independence, while also providing a safeguard for emergency situations. Our staff provide approximately 15 hours a week of support and planned social activities, such as dinner night, game night, movie night, etc.
